The Arc Jefferson - St. Lawrence is one of 35 local Chapters of The Arc New York, the largest provider of supports and services for people intellectual and developmental disabilities in New York state. In addition to the programs The Arc Jefferson - St. Lawrence offers in Jefferson and St. Lawrence County, Chapters of The Arc New York collectively support more than 60,000 people with disabilities and their families in every county of the state, and employ more than 30,000 people in our shared mission to provide people with intellectual, developmental and other disabilities the ordinary and extraordinary opportunities of life.

Fiscal Intermediary/ Self-Directed Support Staff


Position Type/Expected Hours of Work – Flexible to the needs of the program & family (hourly).
 
Qualifications - High School Diploma or GED with one-year experience working with developmentally disabled. Valid acceptable New York State Driver’s License.
 
Work Authorization/Security Clearance – Authorized to work in the United States.
 
Summary/Objective - Provide Community Hab services as indicated on the Individuals Service Plan (ISP) and Community Hab plan. Implement the goals outlined in the Community Hab Staff Action plan. Maintain records of the individuals being served in the Community Hab program and provide them to the Home & Community Services Supervisor weekly. Observe person we support for any unusual physical or behavioral changes and report to supervisor. Attend and satisfactorily complete all required training as assigned. Demonstrate flexibility, accuracy, and confidentiality in work assignments. Maintain confidentiality of all work related information. 
 
Essential Functions –

Implement goals outlined in the Community Habilitation Staff Action plan
Maintain records of the individuals being served in the community Habilitation program and provide them to the Home and Community Services Supervisor weekly.
Observe person supported for any unusual physical or behavioral changes and report to supervisor.
Attend and satisfactorily complete all required training as assigned
Demonstrate flexibility, accuracy, and confidentiality in work assignments.
Maintain confidentiality of all work related information

Travel – Limited travel to client’s home within surrounding counties.

Wage Range - $15.20-$23.00/ Hour, based on the family's budget.
