Internal Support Coordinator


 

Position Summary:

The central role of the Internal Support Coordinator (ISC) is to foster efficient collaboration with teams to ensure the delivery of top-notch services to every person. The ISC holds responsibility for coordinating and documenting programs developed by the team, with a focus on aligning them with the person's individual needs and priorities. Their tasks encompass crafting Individual Service Plan (ISP) programs, conducting observations and assessments, documenting goals and supports, as well as facilitating communication between internal operations and external stakeholders like Conflict Free Case Managers (CFCM), family members, and community resources.

Education and Experience:

§  Minimum of two years of direct experience in supporting people with Intellectual and Developmental Disabilities (IDD).

§  This position prefers a Bachelor’s Degree from an accredited college or university.

Required Skills/Abilities:

§  Proven capability to effectively manage a wide range of responsibilities.

§  Proficiency in budget management and financial oversight.

§  Proficiency in Microsoft Word and Excel.

§  Valid driver’s license with insurability and a clean driving record required.

§  Candidates must have the ability to pass a qualifying criminal background check and drug test.

Physical Requirements:

§  Prolonged periods of sitting at a desk and working on a computer.

§  Must be able to lift 15 pounds at times.

§  Must be able to access and navigate each department at the organization's facilities.

Duties/Responsibilities:

Program Coordination & Development

·       Complete assessments as required.

·        Assist with development of the Individual Service Plans (ISPs) and create ISP programs, as required.

·       Review ISPs and update ISP program based on individual progress and preferences.

·       Facilitate annual ISP meetings with all team members, ensuring active participation from the participant.

·       Utilize a project tracking platform to manage tasks and meet deadlines related to ISP development and implementation.

 

Case Management & Person-Centered Services

·       Establish and maintain relationships with each participant.

·       Serve as a liaison between internal teams and external stakeholders.

·       Coordinate with Conflict Free Case Managers (CFCM).

·       Engage with families and community resources.

 

Communication & Stakeholder Engagement

·       Maintain consistent communication with DSPs, nurses, vocational staff, and other internal team members.

·       Participate in team meetings to review service delivery and individual progress.

·       Respond to internal communications (emails, calls, messages) timely.

·       Attend and contribute to team meetings regularly.

·       Address concerns and unexpected priorities promptly.

 Quality Assurance & Compliance

·       Complete audits of documentation.

·       Prepare and submit required documentation for S.M.A.R.T. audits.

·       Support continuous improvement efforts across service delivery and documentation practices.

·       Document meeting notes showing collaborative planning and follow-up.

 

Financial Oversight & Support

·       Review and reconcile individual finances monthly.

·       Monitor benefits eligibility.

·       Ensure understanding and compliance with agency financial policies and procedures.

·       Provide financial training or support to participants as needed.

 

Rights & Incident Management

·       Address rights restrictions, grievances, and incidents of abuse or neglect per ARSD rules.

·       Ensure all rights restrictions are reviewed and approved through due process procedures.

·       Provide training and guidance to staff on rights, safety, and ethical service delivery.
